New Horizons Medical Center's "Mobile Wellness Unit" has been utilized during the spring and
summer months of 2006 to provide health education and screenings to the communities of Gallatin,
Owen, Carroll and Grant counties.

Screenings have been provided for high cholesterol and hypertension, as well as, testing those
individuals who might be experiencing signs and symptoms of diabetes. New Horizons Medical Center was
also there to help with New Horizons Medical Foundation's Community Wellness Day in April 2006, "Relay for Life"
campaign in June 2006, Elk Lake Health Awareness in July 2006, and the Hispanic Health Fair in Grant County.
Other services include, providing literature on such topics as cancer prevention, childhood
immunizations, safety, diet, and exercise.

New Horizons has screened a total of 246 individuals since the beginning of this year. Encouraging those
individuals with abnormal screening values to seek further treatment. We would like to take this opportunity
